Job summary

HCA Florida Lake Monroe Hospital is seeking a Case Manager to support patients and families through every step of the care journey. You will coordinate services, connect resources, and develop care plans reflecting each patient’s unique needs, partnering with physicians, nurses, and department leaders to ensure safe transitions and high-quality care.

Qualifications

• Licensed as a Registered Nurse in the State of Florida or able to obtain licensure prior to start is required.
• 2 years of RN Case Mgmt. experience in an acute care setting is required.
• BSN is preferred.
• Certification in Case Management, Nursing or Utilization Review is preferred.

Responsibilities

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers.
• Develop individualized treatment plans reflecting patient strengths, needs, and goals.
• Complete psychosocial assessments and discharge planning evaluations; communicate needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless care.
• Promote compassion, respect, and excellence through every patient and family interaction.
