Job summary

HCA Florida North Florida Hospital is hiring an RN Case Manager Days to support patients and families through every step of the care journey. You will coordinate services, connect resources and develop care plans tailored to each patient’s needs, working with physicians, nurses and leaders to ensure safe transitions and clear communication.

You will guide patients through program orientation, coordinate education, develop and implement individualized plans, and document psychosocial assessments

Qualifications

• Bachelor's Degree or RN Diploma or Associate Degree is required.
• RN licensure is required (RN).
• No travel is required for this role.
• Two weekend day shifts per month are required.

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients, families and caregivers.
• Develop individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and document findings clearly.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote HCA Healthcare values in all patient and family interactions.

Education

Bachelor's Degree, or Registered Nurse Diploma, or Associate Degree
