Job summary

HCA Florida Sarasota Doctors Hospital is seeking a Case Manager Registered Nurse to support patients and families through every step of the care journey. You will coordinate services, connect resources, and develop care plans in partnership with physicians, nurses, and department leaders to ensure safe transitions and high-quality outcomes.

Responsibilities include discharge planning, psychosocial assessments, and collaborating with the Rehab Program Director to deliver patient-centered care

Qualifications

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ years in case management or 3+ years in clinical nursing required.
• Licensed RN in state of practice required.
• InterQual experience preferred.
• Certification in Case Management preferred.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients, families, and caregivers.
• Develop and implement individualized treatment plans.
• Complete psychosocial assessments and discharge planning evaluations.
• Collaborate with Rehab Program Director and interdisciplinary team.
• Promote HCA Healthcare values in patient and family interactions.
