HCA Florida Lawnwood Hospital is seeking a Registered Nurse Case Manager to guide patients and families through the care journey, coordinate services, and develop individualized care plans in collaboration with physicians and the care team.

You will support transitions, document psychosocial assessments, and promote the hospital's values of compassion, respect, and excellence while delivering patient-centered care.

Qualifications

• Must hold an RN license.
• Associate Degree in Nursing or RN Diploma required.
• Minimum of 3 years of clinical nursing experience in an acute care hospital setting.

Responsabilités

• Guide patients and families through program orientation, explain rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to support participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs, and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations and communicate needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare’s values of compassion, respect, and excellence in interactions with patients and families.
