Job summary

Curana Health is seeking a Case Manager to join the interdisciplinary care team and help deliver high-quality, patient-centered care across senior living communities. You will assess health and social needs, coordinate plans, and liaise with RP/POAs, PCPs, and care teams to ensure timely, effective care delivery.

The role requires an active RN or LPN license or a BS/MSW degree with clinical practice experience, and it supports a hybrid work model with travel to facilities about 2–3 days per week.

Qualifications

• One year of clinical practice in case management or related areas.
• Basic computer skills and ability to navigate multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Execute on strategic goals set by leadership to improve member experience.
• Conduct comprehensive health assessments with Plan members and develop care plans.
• Educate members and families about disease status, plan benefits, and resources.
• Collaborate with interdisciplinary care teams to ensure appropriate treatment.
• Follow up with members to assess progress toward goals.
• Coordinate with care team and RP/POAs for timely health updates.
• Assist with scheduling appointments and coordinating home health as needed.
• Document care team meetings and transmit updates to Plans.
