Qualifications

• Must be licensed as a Registered Nurse in Massachusetts.
• Minimum three years of nursing experience.
• Three years’ experience in direct care of the elderly or disabled adults.
• Willingness to drive to various locations for home visits.

Responsibilities

• Coordinate health care for individuals in residential homes with the treatment team.
• Supervise clinical operations of the residential home.
• Communicate with Program Directors to obtain and review medical records, current medications, health status, and functional abilities.
• Hire, train, schedule, and supervise LPNs for Rowley Bridge Road.
• Develop and present educational workshops and trainings.
• Ensure staff receive medical and clinical trainings; serve as MAP coordinator.
• Advise on advocacy and ensure care aligns with clinical best practices.
• Conduct comprehensive nursing assessments and develop Case Management Plans with individuals and families.
