Job summary

Northeast Arc is seeking an experienced RN Case Manager to coordinate all health care aspects for residents in our MA residential homes, working with the treatment team to ensure optimal health outcomes. The role supervises clinical operations and supports staff across multiple locations.

Ideal candidates will be fully licensed in Massachusetts with at least three years of nursing experience and three years in direct care of the elderly or disabled.

Qualifications

• Must be a licensed RN in Massachusetts.
• Minimum three years nursing experience and three years in direct care of elderly or disabled adults.

Responsibilities

• Supervises/assists with coordination of health care for individuals in residential homes, collaborating with the treatment team.
• Ensures individuals achieve maximum health and wellness and oversees clinical operations of residential homes.
• Hires, trains, schedules, and supervises LPNs for Rowley Bridge Road.
• Develops and presents educational workshops and trainings for staff.
• Ensures staff receive required medical and clinical trainings; serves as a MAP coordinator.
• Identifies learning needs and secures trainings for staff at various licensure levels.
• Advocates for individuals to ensure care aligns with clinical best practices.
