Job summary

Element Care in Massachusetts is seeking an RN Case Manager to provide comprehensive home-based care coordination for members with complex needs. The role emphasizes development, implementation, and evaluation of individualized care plans, ensuring quality and cost-effective services.

The position requires MA RN licensure, 3–5 years in home care, and access to travel within the local area. Benefits include health, dental, vision, PTO, retirement, and supplemental programs.

Qualifications

• Registered Nurse currently licensed in MA.
• BSN preferred.
• Minimum of three years experience as a licensed, registered nurse, preferably five, including home care clinical experience.
• Proficient in Microsoft Office tools.
• Experience working with case management data systems.
• Ability to travel local on a frequent basis.
• Covid vaccinated preferred.
• Spanish Speaking preferred.

Responsibilities

• Develops a plan of care that corresponds with the physicians’ orders through assessment of patient needs, condition, environment, and consultation with other health team members.
• Ensures the delivery of competent, quality patient care in the home.
• Maintains an ongoing responsibility for assigned caseload.
• Oversees the implementation and evaluation of individual patient plans of care.
• Maintains communication with Commonwealth Care Alliance (CCA) and Element Care staff as well as the provider community as appropriate.
• Provides patient education opportunities.
• Works with both members and their family members.
• Actively participates in team knowledge sharing activities.
• Performs other duties, as assigned.
