CVS Health is seeking a Nurse Case Manager to deliver comprehensive case management services via telephonic and face-to-face interactions. You will assess, plan, implement, and coordinate care strategies to support members' medical needs and promote wellness in accordance with guidelines.

The role requires an active RN license, 3+ years of acute care or case management, 2+ years Med/Surg, and a Monday–Friday schedule from 8:00 am to 4:30 pm in the local time zone. Remote work not specified in ad.

Qualificações

• Active and unrestricted RN license in the state of residence.
• Willing to obtain additional state licenses (paid by the company).
• 3+ years' experience of acute care and/or case management.
• 2+ years' Med/Surg experience.
• Mon-Fri 8:00am-4:30pm local time.

Responsabilidades

• Evaluate member eligibility, identify health risks, and develop proactive care plans.
• Conduct holistic assessments considering comorbid conditions and social determinants of health.
• Review prior claims, evaluate work capacity, and determine referrals to clinical resources.
• Collaborate with supervisors and multidisciplinary teams to optimize care planning.
