SnapCare in Fort Wayne, IN is seeking an RN Case Manager for a 13-week assignment (with extension option). You will conduct utilization reviews, discharge planning, and treatment plan management as part of an interdisciplinary team.

Responsibilities include medical record documentation, patient resource assistance, payor coordination, and timely discharge planning. Requires RN licensure and an associate's degree; Bachelor's degree is preferred. Healthcare insurance knowledge is advantageous.

Qualifications

• Active licensure in a clinical discipline per state guidelines (RN, RT, or SW preferred).
• Associate's degree in a healthcare field required; Bachelor's degree preferred.
• 2 years of Case Management and Discharge Planning experience in Rehab Care setting highly desired.
• Working knowledge of the insurance industry and government reimbursement.

Responsabilités

• Conduct utilization reviews, resource management, discharge planning, treatment plan management, and financial management.
• Complete medical record documentation including plan of care and progress notes per policy and state regulations.
• Identify patient problems or situations affecting the patient and/or family's ability to adapt to or participate in their treatment plan.
• Assist patients and families in utilizing hospital resources to remove barriers to healthcare.
• Collaborate with interdisciplinary team and physicians to manage and direct clinical plans of care to achieve safe and timely discharge.
• Interact appropriately with patients according to age and level of understanding.
• Assure medical necessity review criteria is implemented for all admissions and continued stays.
• Conduct thorough and timely completion of utilization review and non-Medicare utilization.
• Manage payor requirements by ensuring all insurance certifications are completed timely.
• Assure discharge planning evaluations are completed per policy and regulatory standards.
• Maintain up-to-date community resource system and assist patients and families in accessing appropriate services.
• Participate in outlier management and attend weekly meetings as indicated.
• Submit accurate weekly commercial projections to Corporate Finance.
