Job summary

SnapCare seeks an RN for a 13-week assignment on the night shift in Fort Wayne, IN. You will assess patients, develop care plans, and collaborate with the interdisciplinary team to ensure safe, quality care.

This role requires at least two years of bedside experience and current BLS/ACLS certifications. You will participate in rehabilitation activities, discharge planning, and ongoing education while providing clear patient updates to the team.

Qualifications

• Minimum two years of bedside clinical experience.
• BLS and ACLS certifications required before first shift.
• Strong interdisciplinary teamwork and communication.

Responsibilities

• Perform nursing assessments within 8 hours of admission.
• Develop and follow individualized care plans based on data.
• Identify teaching needs with patient, family and team.
• Provide patient information and status updates for team conferences.
• Maintain accurate documentation of assessments, interventions, and responses.
• Participate in the rehabilitation program on the unit.
• Initiate discharge planning and patient teaching from day of admission.
• Communicate changes in status to physicians and charge nurses.
• Provide direction and supervision of LPNs, CNAs, and orientees.
• Assume charge responsibilities after orientation.
