Job summary

IU Health, located at Methodist Hospital, is seeking an RN - Case Manager for a part-time 0.6 FTE role. The position focuses on discharge planning, care coordination, and effective communication with patients, families, providers, and payers to ensure smooth transitions.

This role requires an Associate degree in Nursing, Indiana RN license, and 3–5 years of acute care experience with familiarity in Medicare/Medicaid guidelines. BSN within 5 years of hire is expected for newer RNs.

Qualifications

• Associate degree in Nursing required; BSN must be completed within 5 years for hires after Jan 1, 2013.
• Active Indiana RN license or NLC is required.
• Case Management certification is preferred.

Responsibilities

• Plan and coordinate patient care from pre-admission through post-discharge.
• Develop comprehensive discharge plans and communicate with the care team.
• Coordinate with third-party payers for coverage and authorization.
• Educate patients and families on post-discharge care and safety.
• Ensure compliance with Medicare, Medicaid, and regulatory guidelines.
