Job summary

Qualifications

• Must have graduated from an accredited nursing program.
• Active Indiana RN license is required.
• Case Management experience preferred.
• Hospital experience is preferred.
• Active BLS certification is required.

Responsibilities

• Assess and conducts reviews for medical necessity and level of care.
• Review medical records and communicate with physicians effectively.
• Intervene when determinations are not aligned and refer appropriately.
• Collaborate with UR Committee physicians and care team.
• Support discharge planning and continuity of care with the interdisciplinary team.
• Coordinate plans of care including physical, financial, and psychosocial needs.
