Job summary

Encompass Health Corporation is seeking an RN Case Manager to coordinate patient care from admission through post-discharge. You will guide treatment plans with an interdisciplinary team and manage timely discharge planning while ensuring documentation and regulatory compliance.

The role emphasizes collaboration with clinical staff, payor coordination, and ongoing quality improvement to support meaningful patient outcomes in a rehabilitation setting.

Qualifications

• Qualified to independently complete assessments within scope of practice (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• Active license in the state's discipline.
• CCM® or ACM™ certification required or obtained within two years of entry.

Responsibilities

• Coordinate patient care from admission to post-discharge with interdisciplinary team.
• Attend ACT meetings to identify trends and reduce ACTs.
• Review and document case management plans and addendums.
• Ensure discharge planning is timely and aligned with payor requirements.
• Monitor patient experience and service quality across the care continuum.
• Maintain compliance with regulations, policies, and documentation standards.
• Participate in utilization review data collection and trend analysis.
• Provide timely referrals and information to post-discharge providers.

Education

Associate Degree in Nursing

BSN preferred

Graduate degree preferred

RN licensure

CCM/ACM certification planned
