Heritage Valley Beaver in Beaver, PA is seeking a Nurse Navigator for the Psychiatric Department/ Emergency Room.

The role coordinates evaluations in the Emergency Department and admissions at HVB, overseeing discharge planning and coordinating referrals and aftercare services for patients.

Qualifikationen

• Graduate of an accredited school of nursing with current Pennsylvania license.
• Successful completion of applicable clearances within 90 days of commencing employment.
• CPR certification is required.
• Minimum of 3 years current experience in an inpatient psychiatric setting.
• Knowledge/ability to assess and utilize community resources.

Description

Department: Psychiatric Department/ Emergency Room
Hours: Full-time, 40 hours per week, 8 hours per shift, day/evening coverage, rotating weekends and holidays as required.

Basic Function: This position is responsible for coordination and oversight of evaluations in the Emergency Department and admissions (at HVB only), discharge planning activities, coordinating referrals and aftercare services for patients.

Qualifications:
Required: Graduate of an accredited school of nursing with current Pennsylvania license. Successful completion of applicable clearances as outlined in Human Resources policy HR-106 within 90 days of commencing employment. CPR certification. Must have a minimum of 3 years current experience in an inpatient psychiatric setting. Knowledge/ability to assess and utilize community resources.

Preferred:
Previous experience with psychiatric case management. ANA certification as a psychiatric nurse.
