You will guide patients and families, coordinate education, and develop individualized care plans that reflect each patient’s needs. You will collaborate with clinicians to ensure safe transitions and quality care.

The role requires an active RN license, an Associate Degree, and prior case management experience is preferred. This position is located on site at StoneSprings Hospital Center.

Qualificações

Responsabilidades

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized treatment plans reflecting patient goals.
• Complete psychosocial assessments and discharge planning evaluations.
• Communicate findings across the care team for seamless, patient-centered care.
