Job summary

StoneSprings Hospital Center is seeking a PRN RN Case Manager to support patients and families through the care journey. You will coordinate services, develop care plans, and discharge planning with physicians, nurses, and the care team.

Responsibilities include guiding program orientation, patient education, psychosocial assessments, and collaboration with the Rehab Program Director to ensure safe transitions and high-quality care. Equal opportunity employer.

Qualifications

• RN license in good standing
• Associate degree in nursing or equivalent
• PRN schedule availability
• Prior case management experience preferred
• Located at StoneSprings Hospital Center

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinate education for patients, families, and caregivers to participate in treatment planning and family conferences
• Develop and implement individualized treatment plans reflecting patient strengths and goals
• Complete psychosocial assessments and discharge planning evaluations, document findings clearly for the care team
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless patient-centered care
• Promote HCA Healthcare's values of compassion, respect, and excellence in every interaction
