You will collaborate with physicians, nurses, and the care team to promote patient-centered care, discharge planning, and adherence to rehab principles. The role requires 2 years in Case Management or 3 years in acute care, with comprehensive benefits offered.

Qualifications

• RN credential required.
• 2 years in Case Management or 3 years in acute care.
• Experience coordinating care and discharge planning.

Responsabilités

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families and caregivers.
• Develop and implement individualized treatment plans reflecting patient goals.
• Complete psychosocial assessments and communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote values of compassion, respect, and excellence in care interactions.
