Job summary

StoneSprings Hospital Center, part of HCA Healthcare, is seeking a PRN RN Case Manager to support patients and families through the care journey. You will coordinate services, develop care plans, and work with physicians and nurses to ensure safe transitions and consistent quality.

The role emphasizes discharge planning, patient education, and teamwork with the Rehab Program Director and care team to deliver patient-centered care in a fast-paced environment.

Qualifications

• RN license required as part of Virginia licensure
• Associate degree in nursing or nursing-related credential
• PRN availability or schedule
• StoneSprings Hospital Center location in Virginia, PRN role
• Prior case management experience preferred

Responsibilities

• Guide patients and families through program orientation and discharge planning
• Coordinate education for patients, families, and caregivers
• Develop and implement individualized treatment plans reflecting patient goals
• Complete psychosocial assessments and communicate needs across care team
• Collaborate with Rehab Program Director and interdisciplinary team
• Promote HCA Healthcare values in every interaction
