Responsabilités

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers.
• Develop individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and discharge planning evaluations.
• Collaborate with Rehab Program Director and care team for seamless patient-centered care.
