Job summary

HCA HealthONE Mountain Ridge is seeking an RN Case Manager PRN to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans in partnership with physicians, nurses, and the care team.

The position emphasizes safe transitions, clear communication, and high-quality, patient-centered care. Active RN licensure and 2 years case management or 3 years acute care experience are required, with a comprehensive benefits

Qualifications

• RN licensed with an active Colorado license.
• Minimum 2 years in case management or 3 years in acute care.
• Strong communication and care coordination skills.

Responsibilities

• Guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Developing and implementing individualized treatment plans reflecting patient strengths and needs.
• Completing psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborating with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care.
• Promoting HCA Healthcare’s values of compassion, respect, and excellence in every interaction.
