Job Summary and Qualifications

We seek candidates with hospital nursing experience; case management experience is preferred but not required.

Shifts are 8-4:30 (Mon-Fri), with opportunities for weekend/holiday shifts. We seek someone willing to work at least 1x per week and be available PRN for staffing needs.

The Case Manager ensures high-quality, patient-centered rehabilitative care, coordinating efforts within the rehab team and collaborating with leadership.

Responsibilities include:

• Providing orientation to patients, families, and caregivers about the case manager role, rehab philosophy, eligibility, Medicare, insurance, grievance procedures, treatment plans, and patient rights.
• Coordinating patient/family/caregiver education and promoting participation in care planning through conferences and interactions.
• Ensuring implementation of treatment plans tailored to patient needs and preferences, facilitating patient involvement.
• Documenting discharge planning evaluations and psychosocial assessments, communicating social, financial, and discharge needs.
• Promoting positive patient interactions and service excellence.

Qualifications:

• Registered Nurse (RN)
• Diploma or Associate Degree in Nursing
