Penn Presbyterian Medical Center (PPMC) offers an outstanding opportunity for a PRN RN Case Manager to join the Case Management team and work alongside some of the best medical providers in Philadelphia.

Responsibilities include:
  • Monitoring appropriate use of internal resources and communication with the interdisciplinary care team and Physician Advisor.
  • Coordinating discharge planning activities such as home care, DME, placements, hospice, community resources, transportation, and quality data collection.
  • Working with patients and families to ensure post-hospital care options align with their preferences.
  • Educating patients about healthcare and insurance benefits, respecting privacy and dignity.
  • Communicating expected discharge timeframes and staying updated on new providers, technologies, and resources.
  • Providing developmentally appropriate interactions with older adult patients.

Discharge Planning:

  • Assessing social, emotional, and discharge needs of patients and families.
  • Developing and facilitating care plans addressing identified concerns.
  • Documenting delays and communicating issues effectively.
  • Ensuring smooth transitions to appropriate care settings and community resources.
  • Participating in interdisciplinary team meetings and driving accountability.

Utilization Management:

  • Adhering to guidelines to meet third-party payer requirements.
  • Educating the team on documentation standards and addressing care level issues proactively.
  • Accurately documenting review activities in Canopy and managing case statuses.

Quality Improvement and Risk Management:

  • Collecting and submitting data, communicating risk issues, and collaborating on solutions.

Technology and Systems:

  • Staying current with new systems (MedView, On Base, SafetyNet, Sunrise, etc.) and incorporating technology into daily work.

Credentials:

  • Active RN PA license in good standing required.

Education and Experience:

  • Bachelor's Degree in Nursing required.
  • 2+ years of clinical experience preferred.
