What You’ll Do:

• Conduct assessments and create personalized care plans
  
  
• Monitor and adjust treatment based on client needs
  
  
• Educate patients and families on care instructions and follow-ups
  
  
• Perform supervisory visits every 90 days (or every 60 days for CAP cases)
  
  
• Collaborate with home health aides and field staff to ensure quality care
  
  
• Travel to client homes in High Point, Archdale, Thomasville, and surrounding areas
  
  
• Keep accurate records and report changes in client condition
  
  

Requirements:

• Active RN license in NC or compact/multi-state license
  
  
• Graduate of an accredited nursing program
  
  
• Minimum of 2 years of nursing experience (home health/case management a plus!)
  
  
• Comfortable using EMR systems and mobile documentation
  
  
• Reliable transportation and flexibility to travel for visits
  
  
• Strong communication and clinical judgment skills
  
  
• A heart for home care and patient-centered service
