Qualifications

What you will do in this role:

  • Complete departmental orientation, initial and annual competencies.
  • Collaborate with other departmental staff to coordinate patient care, identify barriers to care or discharge, and develop solutions.
  • Document per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
  • Schedule family conferences and communicate with caregivers to keep them informed of progress, goals, and discharge plans.
  • Coordinate weekly patient care team conferences to develop, monitor, and refine treatment plans.
  • Review the patient’s assigned CMG, identify potential missed comorbid conditions, and communicate findings to the HIM team.
  • Perform other duties as assigned.

Qualifications:

  • Current licensure as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
  • Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
  • Minimum of 2 years of social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.
  • Effective communication skills in English; additional languages preferred.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
