Department: System Case Management

Schedule: PRN scheduled on an as-needed basis; with additional opportunities to pick up shifts in other St. Louis locations.

Requirements: Must have 3 years of acute care experience; case management experience highly preferred.

Job Summary:

Assesses, develops, plans, implements, and evaluates patients' discharge planning needs to the next level of care in collaboration with an interdisciplinary team.

Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
  • Ensures compliance with Condition of Participation (CoP) discharge planning.
  • Identifies and escalates gaps/barriers related to inpatient and observation length of stay.
  • Assesses patients at high risk for readmission, identifies root causes, and develops plans in collaboration with the multidisciplinary team.
  • Participates in team discussions regarding opportunities for process improvement.
  • May be responsible for utilization management, ED case management, admission processes, and acting as a liaison with post-acute providers.
  • Applies evidence-based practices and scientific knowledge to nursing care, ensuring delivery based on patients' age-specific and clinical needs.
  • Demonstrates professional nursing standards per the practice model and uses the ANA Code of Ethics.
  • Works safely and performs other duties as assigned.
Education
  • Graduate of an accredited nursing school or equivalent licensing.
Experience
  • Minimum of three years' acute care experience.
Physical Requirements
  • Frequent lifting, carrying, sitting, standing, walking, reaching, and repetitive movements.
  • Use of vision, hearing, speech, and keyboard for data entry.
  • Occasional bending, stooping, kneeling, and lifting heavier objects.
Licenses and Certifications

Registered Nurse (RN) license for Illinois, Missouri, Oklahoma, or Wisconsin, depending on location, issued by the respective state board or through a compact state agreement.
