Job Responsibilities:

  • Complete departmental orientation, initial, and annual competencies
  • Collaborate with departmental staff to coordinate patient care, identify barriers, and develop solutions
  • Document care activities per workflow timelines, including the Individual Plan of Care (IPoC) per CMS guidelines
  • Schedule family conferences and communicate with caregivers to keep them informed of patient progress and discharge plans
  • Coordinate weekly patient care team meetings to develop and monitor treatment plans
  • Review patient data to identify any missed comorbid conditions and communicate findings to the HIM team
  • Perform other duties as assigned

Qualifications:

  • Current licensure as a Registered Nurse, Social Worker, or other healthcare professional (Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, Occupational Therapist)
  • Preferred certifications include CCM, ARN, ACM, or RN-BC
  • Minimum of 2 years of social work or case management experience in an inpatient setting; rehab experience preferred
  • Effective communication skills in English; additional languages a plus
  • Basic computer skills in MS Office suite
