Qualifications

• Qualified registered nurse per state Practice Act.
• CPR certification.
• Maintains licensure and continuing education.
• Ability to supervise ancillary disciplines per state requirements.

Responsibilities

• Develops, delivers, implements, revises, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and ancillary staff.
• Submits required data collection sets (OASIS, PAR, etc.) within required timeframes.
• Performs initial evaluation visits and regular nursing reevaluations of patient needs.
• Coordinates care between patients, families, agencies and other providers.
• Administers medications per agency policy and physician orders.
• Attends case conferences and reports changes in patient condition to the Nursing Supervisor.
