Job summary

Home Health is seeking an RN Case Manager to oversee patient care visits and coordinate multidisciplinary care, serving as the primary contact for the care team. You will manage visit frequency for each episode and consult with physicians as needed to ensure quality patient care.

Responsibilities include accurate OASIS collection, up-to-date medication profiles, timely lab reporting, weekly case conferences, and thorough documentation of patient history and assessments.

Qualifications

• Must hold a current, unencumbered RN or LPN license.
• CPR certification required.
• Prior clinical experience is essential.
• Excellent communication skills.
• Demonstrated professionalism during stressful times.

• Ability to adapt to different clinical settings and environments.

Responsibilities

• Perform visits and coordinate client care with multiple disciplines.
• Act as the point of contact for all care disciplines and oversee visit frequency.
• Consult with physicians as needed and provide care updates.
• Accurately perform OASIS collection and keep medication profiles current.
• Ensure lab values are reported to the physician timely.
• Attend weekly case conferences and monthly case manager meetings.
• Document patient history, symptoms, assessments, tests, and care per guidelines.
