Job Summary:

Assesses, develops, plans, implements, and evaluates patients' discharge planning needs to the next level of care in collaboration with the interdisciplinary team.

Job Responsibilities and Requirements:
Primary Responsibilities
  • Ensure compliance with Condition of Participation (CoP) discharge planning.
  • Identify and escalate gaps or barriers related to inpatient and observation length of stay.
  • Assess patients at high risk for readmission, identify root causes, and develop plans in collaboration with the multidisciplinary team regarding discharge to the next level of care.
  • Participate with other team members in opportunities for improvement in standard work.
  • May also be responsible for utilization management, ED case management, admission processes, and acting as a liaison with post-acute providers (skilled nursing facilities, rehab, home health, etc.).
  • Apply evidence-based practice and scientific knowledge in health care to nursing practice, ensuring care delivery based on patient’s age-specific and clinical needs as per the department's Scope of Service.
  • Demonstrate professional nursing standards according to the professional practice model, guided by the ANA Code of Ethics.
  • Maintain a constant state of alertness and work safely.
  • Perform other duties as assigned.
Education:
  • Graduate of an accredited school of nursing or equivalent for licensing.
Experience:
  • Three years of acute care experience.
Physical Requirements:
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive movements.
  • Use of vision, depth perception, and color recognition.
  • Use of hearing and speech for communication and alarm detection.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.
Required Licenses/Certifications:

State-specific RN licensure (Illinois, Missouri, Oklahoma, Wisconsin) as detailed in the original description.
