Job Description: Discharge Planning Nurse (PRN)

Job Highlights:

  • Schedule: PRN, as needed
Job Summary:

Assess, develop, plan, implement, and evaluate patients' discharge planning needs for the next level of care in collaboration with the interdisciplinary team.

Responsibilities:
  • Ensure compliance with Condition of Participation (CoP) discharge planning.
  • Identify and escalate gaps or barriers related to inpatient and observation length of stay.
  • Assess patients at high risk for readmission, identify root causes, and develop discharge plans collaboratively.
  • Participate in team discussions on process improvements.
  • May handle utilization management, ED case management, admission processes, and act as liaison with post-acute providers.
  • Apply evidence-based practice and scientific knowledge to nursing care based on patient age and clinical needs.
  • Adhere to professional nursing standards, including the ANA Code of Ethics.
  • Maintain safety and alertness at all times.
  • Perform other duties as assigned.
Qualifications:
  • Graduate of an accredited nursing program or equivalent.
  • Minimum of three years' acute care experience.
Physical Requirements:
  • Frequent lifting/carrying of objects 0-25 lbs and occasional 25-50 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive movements.
  • Use of vision, depth perception, hearing, and speech.
  • Frequent data entry; occasional bending, stooping, kneeling, and lifting.
Licenses and Certifications:
  • Valid RN license in Illinois, Missouri, Oklahoma, or Wisconsin, as applicable.
Work Schedule:

PRN / Per Diem, as needed
