Job Summary

Coordinates and performs interdisciplinary care plans by assessing patient conditions, traveling to patient homes, facilities, or hospitals to meet patient and family needs, and ensuring compliance with policies and regulations.

Primary Responsibilities

• Assess and coordinate discharge planning needs.
• Perform point of care testing as per policies.
• Collaborate with physicians and healthcare team, update on patient progress, and document accordingly.
• Coordinate with outside agencies, ensure documentation, and develop comprehensive care plans.
• Complete admission paperwork, verify insurance, and perform medical assessments.
• Provide skilled nursing care including vitals, wound care, IV care, etc.
• Document visits and treatments accurately and timely.
• Develop care plans with patients and families, respond to calls and follow-up on needs.
• Maintain safety and perform other duties as assigned.

Education & Experience

• Graduate of an accredited nursing school or equivalent licensing.
• No experience required.

Physical & Professional Requirements

• Constant use of speech, standing, walking, and frequent lifting of 0-25 lbs.
• Frequent sitting, reaching, vision, hearing, and driving.
• Occasional lifting of 25-50 lbs and patient movement.
• Valid RN license in the state of work, BLS certification, and a clean MVR.
