Job Description

INTEGRIS Health Baptist Medical Center, Oklahoma’s largest not-for-profit health system, has a great opportunity for an RN Case Manager in Oklahoma City, OK. This full-time position (8:00am-5:00pm) involves working with our Case Management team to provide exceptional care. The Case Manager is responsible for clinical and financial outcomes for an assigned caseload, facilitating patient care during hospitalization, managing resource utilization, and coordinating patient movement across care continuum. The role involves acting as a clinician, educator, researcher, manager, and advocate, adhering to safety goals and regulations.

Responsibilities
  1. Conduct comprehensive assessments of patients’ clinical, psychological, and financial needs.
  2. Coordinate timely transfers to appropriate care levels based on clinical needs and utilization criteria.
  3. Develop and revise discharge plans, including referrals to healthcare and community resources.
  4. Communicate care plans and changes to patients, families, and healthcare professionals.
  5. Assist with resource utilization and discharge planning, facilitating timely patient discharges.
  6. Review patient records concurrently and apply utilization criteria accurately.
  7. Notify relevant parties of utilization issues affecting care or reimbursement.
  8. Facilitate patient transfers in accordance with policies and regulations.
  9. Advise physicians on discharge planning, documentation, and resource utilization.
  10. Maintain knowledge of CMS, Medicare/Medicaid, managed care, and payer regulations.
  11. Educate patients, families, and healthcare staff on relevant regulations and benefits.
  12. Identify cost reduction opportunities without compromising quality.
  13. Participate in data collection for quality improvement.
Mental Health Specific Responsibilities
  1. Lead age-appropriate patient education and recreational groups.
  2. Document group activities and facilitate discharge planning.
  3. Participate in treatment team meetings and develop patient care plans.
  4. Assist with insurance authorization and serve as a patient advocate.
  5. Develop and facilitate psychotherapeutic groups and monitor medications.
  6. Assess patient suitability for admission and monitor vital signs.
INTEGRIS Certified Case Management Responsibilities
  1. Document patient progress and communicate challenges timely.
  2. Manage schedules and act as liaison between patients, providers, and the organization.
Qualifications
  • Minimum 5 years of clinical experience in various settings.
  • Experience with managed care and payer requirements preferred.
  • Current RN licensure in Oklahoma or multistate license (eNLC).
  • BSN preferred.
  • Strong interpersonal and computer skills.
  • Case Management Certification preferred, or to be obtained within one year of hire.
  • Valid Oklahoma Driver's License.
