Job summary

Precision-Healthcare in Milford, MA seeks an RN Case Manager to support adult and geriatric patients through case management and discharge planning.

This 10-week contract offers 40 hours per week with dayshift, and a gross weekly rate up to $2,300 plus benefits from day one. Requirements include MS RN licensure in MA, BSN, acute care experience; Epic preferred.

Qualifications

• 3–5 years of nursing experience required.
• BSN required.
• Massachusetts RN license required.
• Acute care experience required.
• Case management and utilization review experience.

Responsibilities

• Provide case management services for adult and geriatric patients.
• Coordinate patient care and discharge planning.
• Complete patient needs assessments and coordinate DME orders.
• Support care coordination across the acute care setting.
• Apply CMS and HIPAA guidelines to patient care and documentation.
• Collaborate with interdisciplinary teams to support safe and appropriate patient outcomes.

Location: Milford, MA

Rate: Up to $2,300 Gross Weekly + benefits available from day 1

Shift: Days, 5x8-hour shifts, 8:00 AM-4:30 PM; every third weekend required

Contract: 10 weeks

Hours: 40 hours/week

Requirements:

• 3-5 years of experience required
• BSN required
• Massachusetts RN license required
• Acute care experience required
• Case management/utilization review experience
• Care coordination and discharge planning experience
• Epic experience preferred
• Local and travel candidates accepted

Certifications:

• BLS required
• CCM preferred
