Job summary

RN.com is seeking an RN Case Manager to coordinate patient care from admission through discharge in an acute hospital setting. You will collaborate with physicians, nurses, and external providers to remove barriers and support safe, efficient transitions.

Requirements include TX/compact RN license, BLS, and 1+ year of case management experience within a hospital context. 13-week assignment with 8:00 AM–4:30 PM shifts.

Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS –

• TX/compact RN license
• BLS
• 1+ year case management experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break

PREFERRED QUALIFICATIONS –

• CCM
• Meditech & Careport experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8A-4:30P
