Job summary

RN.com is seeking a skilled RN Case Manager in Missouri/Kansas City to coordinate patient care from admission through discharge, ensuring alignment of treatments with each patient’s needs. The role requires MO/compact RN license and BLS, with at least 2 years of case management experience in an acute hospital setting and recent discharge planning

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS –

• MO/compact RN license
• BLS
• 2 years case management experience in an acute hospital setting
• Acute hospital experience in specialty during the past year with no more than a 90-day break
• Recent acute discharge planning experience

LENGTH OF ASSIGNMENT – 12 weeks

SHIFT / HOURS PER WEEK – M-F 8-4:30 rotating weekends

SYSTEMS – Meditech, Midas, Wellsky
