Job summary

RN.com is seeking an RN Case Manager to coordinate patient care from admission through discharge, ensuring treatments align with each patient’s medical needs and recovery goals.

Work with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. 13-week assignment with a fixed shift schedule.

Qualifications

• FL/compact RN license required.
• BLS certification required.
• 2 years case manager experience in an acute hospital setting.
• Acute hospital experience in a specialty during the past year with no more than a 90-day break.
• Knowledgeable in Meditech, Midas, and Interqual.

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS –

• FL/compact RN license
• BLS
• 2 years case manager experience in an acute hospital setting
• Acute hospital experience in specialty during the past year with no more than a 90-day break
• Knowledgeable in Meditech, Midas, and Interqual.

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00AM-4:30PM with every other weekend

SYSTEMS – Meditech, Midas, Interqual
