Job summary

Abby Care is seeking a dedicated Registered Nurse to join our team as an RN Case Manager in the Merrimack Valley/Lawrence area of Massachusetts. This is a full-time role reporting to the Director of Nursing.

You will coordinate patient care, collaborate with PCPs and therapy providers, and mentor Home Health Aides while maintaining regulatory standards. On-call rotation and weekend coverage may be required.

Qualifications

• MA RN license required.
• 2 years RN experience in a clinical setting.
• 1 year home health experience preferred.
• Case management/supervisor experience preferred.
• Knowledge of Medicaid paid family caregiving program and HHA scope of practice preferred.
• Reliable transportation and valid drivers license required.

Responsibilities

• Manage a diverse caseload and coordinate patient care.
• Coach and supervise Home Health Aides to ensure quality care.
• Develop and implement individualized care plans.
• Coordinate with PCPs, therapy providers, and internal teams.
• Educate patients and families about health care needs and plans.
• Maintain timely electronic charting and regulatory compliance.
• Participate in on-call rotation, weekends, and holidays.
