Job summary

Newbridge Recruiting Group LLC in Lynn, MA is seeking an experienced RN Case Manager for a hybrid role with a predictable schedule and limited on-call. You’ll conduct in-home assessments, coordinate care, and support older adults to stay safely at home.

This position offers Monday–Friday 8:00 AM–4:00 PM, 2 remote days, strong interdisciplinary collaboration, and meaningful patient relationships. A Massachusetts RN license and 3+ years of nursing experience are required.

Qualifications

• Active Massachusetts RN license.
• 3+ years of nursing experience; 5+ years preferred.
• Experience in case management, home care, community health, geriatrics, or care coordination is a plus.
• Strong clinical judgment and critical-thinking skills.
• Excellent communication and organizational abilities.
• Reliable transportation for local travel.

Responsibilities

• Conduct comprehensive in-home assessments
• Develop and manage individualized care plans
• Manage a caseload of older adults with complex medical and social needs
• Coordinate services among physicians, specialists, caregivers, and community resources
• Educate patients and families on care needs, resources, and available services
• Monitor progress and identify changes in health or care needs
• Maintain accurate clinical documentation and ensure compliance
• Collaborate with an interdisciplinary team to improve patient outcomes
