Job summary

Abby Care is seeking a dedicated Registered Nurse to join our team as an RN Case Manager based in Massachusetts. This full-time role reports to the Director of Nursing and involves coordinating care for patients, collaborating with PCPs and therapy providers, and supporting home health services.

Strong organizational and communication skills are essential. The ideal candidate will have an active MA RN license, 2 years of clinical RN experience (home health preferred), and willingness to travel

Qualifications

• Active Massachusetts RN license.
• 2 years of experience as a Registered Nurse in a clinical setting; 1 year of home health experience preferred.
• Case management/supervisor experience and pediatric/adult care experience preferred.
• Spanish language skills preferred.
• Reliable transportation, valid driver's license, and current auto insurance required.
• Ability to work flexible hours, travel locally, and provide on-call service to patients/families.
• Knowledge of OASIS preferred (training provided if needed).

Responsibilities

• Manage a diverse caseload with high-quality patient care and coordination.
• Coach, supervise, and support Home Health Aides to ensure clinical standards.
• Develop and implement individualized care plans based on assessments and goals.
• Coordinate with PCPs, therapy providers, and internal teams to ensure comprehensive care.
• Provide education and support to patients and families regarding health care needs and treatment plans.
• Maintain accurate electronic charting with timely documentation (24-hour turnaround).
• Escalate issues to Clinical Leads for order management and patient concerns.
• Participate in on-call rotation (about one weekend per month) and holiday coverage.
