Job summary

Right at Home Boston Northwest is seeking an experienced Registered Nurse (RN) Case Manager for field-based home care across the Boston Northwest area, including Bedford, Lexington, Burlington, Woburn and surrounding communities. Local travel between client homes is required, with supervision from the Director of Nursing and care team.

The role emphasizes clinical assessment, care coordination, and building relationships with families, caregivers, and partners, offering professional independence

Qualifications

• Active, unrestricted RN license in Massachusetts.
• 2+ years of nursing experience.
• Home care, geriatrics, private-duty nursing, or case-management experience preferred.

Responsibilities

• Conduct in-home nursing assessments and reassessments for clients.
• Develop, review, and update plans of care.
• Provide ongoing clinical case management and follow-up visits.
• Coordinate care with physicians and other healthcare professionals.
• Educate and support caregivers in delivering safe, high-quality care.
