Abby Care in Worcester County North/Fitchburg Area is seeking a Registered Nurse to join as an RN Case Manager. The role reports to the Director of Nursing in Massachusetts and focuses on comprehensive care coordination and patient outcomes.

The ideal candidate will have an RN license in MA, 2 years of clinical RN experience, and home health/case management exposure. Bilingual Spanish is preferred; travel and on-call duties may be required.

Qualifications

• Requires an RN license in Massachusetts, with current valid credentials.
• Experience in home health and case management is preferred.
• Spanish language ability is a plus.
• Knowledge of Medicaid paid family caregiving program and HHA scope of practice is preferred.
• Flexibility to travel locally and provide on-call coverage.
• Reliable transportation and valid driver's license required.

Responsibilities

• Manage a diverse caseload ensuring quality patient care and coordination.
• Coach and supervise Home Health Aides to uphold clinical standards.
• Develop individualized care plans based on assessments and goals.
• Collaborate with PCPs, therapy providers, and internal teams to ensure comprehensive care.
