AccentCare is seeking an RN Case Manager in Mississippi covering Jasper and Newton Counties. The role manages patient care plans from admission through discharge and coordinates nursing staff to ensure quality patient care. Schedule is Monday to Friday, 8:00am–4:30pm, with on-call duties every 5–7 weeks.

Requirements include RN licensure in MS, at least 1 year of nursing experience, and CPR certification. Compensation includes a $5,000 bonus and a pay range of $71,000–$79,000, scaled per-point

Qualifications

• RN license in MS and in good standing
• Minimum 1 year of RN experience
• Current CPR Certification

Responsabilités

• Assess status of nursing needs and manage patient care plan
• Complete initial patient assessment on admission with history
• Select nursing diagnoses for patients
• Review medications for adverse effects and contraindications
• Set realistic, measurable goals aligned with diagnosis
• Inform physician of assessment results and interventions
• Implement nursing interventions within the Nurse Practice Act
• Re-evaluate patient needs as required
• Document assessments, progress and discharge notes timely
• Inform physician of changes in patient condition
