Job summary

AccentCare, Inc. is seeking an RN Case Manager to oversee care plans from admission to discharge in the Jasper & Newton Counties area of MS. The role combines direct and indirect patient care, directing staff and coordinating allied health clinicians to ensure quality outcomes.

The position requires RN licensure in the state of operation, CPR certification, and a minimum of one year of RN experience. Schedule is M-F with on-call duties every 5–7 weeks, and compensation is paid on a per-point

Qualifications

• RN license in the state of operation required
• Minimum of one year of RN experience
• Current CPR Certification required

Responsibilities

• Assess the status of nursing needs and manage the patient's care plan
• Complete initial patient assessment upon admission, including history
• Identify appropriate nursing diagnoses and document progress
• Coordinate with physicians and interdisciplinary teams for interventions
• Re-evaluate patient needs and update care plans as required
• Document assessment, progress, and discharge notes timely
