Job summary

AccentCare, Inc. is seeking an experienced RN Case Manager to oversee patient care plans from admission to discharge in Rank County, MS. The role combines direct and indirect care, coordinating with nurses, LPNs, and CHHAs to deliver the plan of care.

The ideal candidate holds an RN license in Mississippi, with at least one year of RN experience and CPR certification. Bonus package and full-time hours are offered, with a work schedule of 8:00am-4:30pm and periodic on-call requirements.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of operation.
• A minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsibilities

• Assess nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission with history.
• Select appropriate nursing diagnosis.
• Review medications for therapy issues and allergies.
• Set realistic goals aligned with diagnosis.
• Inform physician of assessment results and interventions.
• Implement nursing interventions within Nurse Practice Act.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Inform physician of changes in patient condition.
