Alternate Solutions Health Network seeks an experienced RN Case Manager to lead home health care in the Kettering area. You will assess patients, develop and coordinate plans of care, educate families, and ensure compliance with physician orders and regulatory guidelines.

You will supervise care delivered by nurses and aides, perform OASIS assessments, monitor patient conditions, and collaborate with physicians to reduce hospital readmissions while maintaining high-quality, cost-effective care.

Formación

• Minimum of one year of RN experience in acute care; home health preferred.
• Ability to communicate effectively with patients, families and physicians.
• Knowledge of nursing best practices and eligibility for home health supervision.

Responsabilidades

• Oversees overall patient care from start to end of episode.
• Updates physician orders in patient chart and notifies changes.
• Performs OASIS assessments to tailor plans of care.
• Administers medications as prescribed and educates family members.
• Coordinates multi-disciplinary care and reduces rehospitalizations.
• Supervises nurse aides and LPNs, ensures chart documentation.
• Maintains compliance with Medicare/agency guidelines.
