Job summary

Alternate Solutions Health Network seeks an experienced RN Case Manager to lead patient assessments, plan of care, and multidisciplinary coordination in home health. You will educate patients and families, ensure documentation accuracy, and maintain compliance with regulations.

The RN educates patients and families, ensures safety, and coordinates care under physician direction. A valid RN license and state compliance are required for successful practice.

Qualifications

• Minimum of one year of RN experience in acute care.
• Home care experience is preferred.
• Strong communication with patients and families.
• Ability to coordinate multi-disciplinary care.
• Valid driver’s license required.

Responsibilities

• Oversees patient care from start to end of episode.
• Updates doctor orders in patient chart.
• Notifies doctor of significant changes in patient condition.
• Provides skilled nursing care in patient homes as ordered by physician.
• Completes clinical documentation per Medicare/Medicaid guidelines.
• Drives to patient locations and attends scheduled visits.
