Job summary

teamselecthh is seeking a Registered Nurse (RN) to support home health case management and skilled nursing needs in patients' homes. The role blends hands-on nursing with intermittent case management, ensuring high-quality care per physician orders and regulatory requirements.

The RN will perform initial evaluations, develop care plans, submit required documentation such as OASIS/PAR, and coordinate with patients, families, and providers. On-call duties may apply as part of the clinical team.

Qualifications

• Licensed Registered Nurse (RN) in good standing.
• Experience with home health care and case management preferred.
• Knowledge of state Practice Acts and regulatory requirements.
• Ability to manage care plans and coordinate with families and providers.

Responsibilities

• Develops and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and staff.
• Submits required data (OASIS, PAR) timely for state/federal payers.
• Performs initial and ongoing patient evaluations and reassessments.
• Coordinates care among patients, families, and healthcare providers.
• Administers medications per orders and maintains documentation.
• Participates in conferences and reports changes in patient condition.
• Maintains confidentiality and adheres to agency policies.
