Alternate Solutions Health Network is seeking a Registered Nurse Case Manager to lead patient assessment, plan of care, and coordination of multidisciplinary care in patients' homes. You will educate patients and families, ensure safety, maintain compliance, and document all activities under physician direction.

Qualifications

• Active RN license in the state of employment.
• Minimum of one year of experience as an RN in an acute care setting.
• Home care experience is desirable.

Responsibilities

• Oversees patient care from start to end of episode.
• Updates doctor orders and notifies for changes in condition.
• Manages multidisciplinary care and provides skilled nursing at home.
