Baptist Memorial Health Care Corporation is seeking a Case Manager to coordinate clinical, psychosocial and financial aspects of patient care. The role emphasizes collaboration with physicians, nurses, social workers and payors to ensure safe discharge and effective transitions.

Minimum 3 years of healthcare acute care experience and an accredited Nursing program graduate are required; RN licensure in MS is preferred with case management certification as a plus.

Qualifications

• Minimum 3 years in healthcare/medical acute care.
• RN license required (State MS).
• Case management certification preferred.

Responsabilités

• Assess and document clinical, psychosocial and financial needs of patients.
• Apply InterQual to determine Level of Service and Intensity of Care.
• Identify plan of care, estimated length of stay and discharge plan with physicians.
• Coordinate with nursing and social work to ensure transition and avoid gaps in care.
• Ensure timely discharge/transition and document variances.
• Escalate barriers to appropriate treatment or transition.
• Maintain knowledge of facilities and resources for patients and caregivers.
